Why men should care about HPV vaccination

Until recently, HPV vaccination was widely (and inaccurately) understood in the Philippines as 'something for young girls to prevent cervical cancer.' That framing has been corrected by every major clinical body — ACIP in the US, the European Society of Medical Oncology, and the WHO — but the cultural lag persists in Manila clinical practice. The current picture for adult Filipino men:

HPV-related cancers in men are increasing globally and in the Philippines. Oropharyngeal cancer (the back of the throat, tonsils, base of tongue) is now the most common HPV-related cancer in men in many countries, surpassing cervical cancer in women in some populations. Anal, penile, and head-and-neck cancers are all linked to high-risk HPV types.

Genital warts caused by HPV types 6 and 11 are common in Filipino men and are a frequent reason for clinic visits at our and other discreet practices. Vaccination prevents these — both for the vaccinated individual and for partners through reduced transmission.

Vaccination protects future partners. Reducing HPV circulation in the population is how cervical cancer rates drop in countries with high adult vaccination coverage.

What vaccine is available in the Philippines

The current adult HPV vaccine in the Philippines is Gardasil 9 (9-valent recombinant HPV vaccine), manufactured by Merck. It protects against nine HPV types:

Together these nine types account for approximately 90% of HPV-related cancers and the vast majority of anogenital warts globally. The 2-valent (Cervarix) and 4-valent (original Gardasil) formulations have been largely replaced by Gardasil 9 in Philippine adult practice.

Gardasil 9 is FDA Philippines-approved for adult vaccination and is the standard offering at private clinics including ours.

Age recommendation — and why 'after 26' is still recommended

Current Philippine and ACIP-aligned recommendations for HPV vaccination by age:

The 'after 26' recommendation matters because it corrects the older guidance (which capped vaccination at 26) that had quietly written off adult men. The current evidence is clear: vaccination through age 45 confers protection against the HPV types the patient has not been exposed to. Most adults have been exposed to a smaller number of HPV types than they assume — exposure to one type does not prevent infection with the others.

If you are between 27 and 45 and have not been vaccinated, the conversation at consultation is about your specific situation: new or anticipated new sexual partners, history of genital warts, prior HPV testing, and current partner status. For most adult Filipino men in this age range with any of those circumstances, the cost-benefit favours vaccination.

Dosing schedule — the practical details

For adults aged 15 and over, the standard 3-dose schedule is:

If a dose is missed or delayed, the series does not restart — you continue from where you left off. The minimum interval between dose 1 and dose 2 is 4 weeks; between dose 2 and dose 3 is 12 weeks; total course should be completed in 12 months when possible. Vaccines are administered intramuscularly, typically in the deltoid (shoulder).

All three doses are required for the full immune response. Two doses provide partial protection; one dose substantially less, though some recent data suggests one-dose protection is more durable than originally thought.

Realistic Philippine pricing

Adult HPV vaccination is private pay in the Philippines — PhilHealth does not cover it for adults, and most HMOs categorise it as preventive elective. Typical Manila private-clinic pricing in 2026:

If pricing is a barrier and you'd otherwise not vaccinate, the lower-cost end of the market is reasonable provided the vaccine is the genuine Gardasil 9 (ask to see the box and lot number) and the storage is cold-chain compliant. Beware of unusually cheap offerings from non-registered providers — Gardasil 9 has a meaningful cost-of-goods that limits how low legitimate pricing can go.

Side effects and what to expect at the visit

The HPV vaccine has been extensively studied across more than 15 years of clinical use and has an excellent safety profile. Common side effects (most resolve within 1–2 days):

Serious adverse events are exceptionally rare. The vaccine is contraindicated in patients with severe allergy to yeast (vaccine is recombinant in yeast) and should be deferred during acute moderate-to-severe illness or pregnancy.

At the clinic visit, expect: a brief medical history review, confirmation that you're not pregnant (where relevant), confirmation of no significant yeast allergy, intramuscular injection in the deltoid, and 15 minutes of post-injection observation. Total visit time: 25–30 minutes for the first dose; 15–20 minutes for subsequent doses.
