How each one works
RF (radiofrequency). An RF probe passes electrical energy into the tissue, which meets natural resistance and produces volumetric — or "bulk" — heating of the vaginal mucosa and the layer beneath it, bringing it to a therapeutic temperature. That controlled warmth signals the tissue's collagen-making cells to remodel and firm over the following weeks. RF is non-ablative, so the surface stays intact. Protocols vary by device: some are a single session, but many use a short series (commonly three monthly sessions) followed by maintenance, and some RF platforms also treat the external labial area.
HIFU (high-intensity focused ultrasound). A specialised intravaginal probe concentrates ultrasound energy at precise depths — typically 3.0 mm and 4.5 mm — creating microscopic thermal coagulation points below the surface while leaving the mucosa itself intact. The same collagen-and-elastin remodeling follows. Because the surface is never broken, there's no open wound and essentially no downtime. This is the mechanism behind the Reju-V HIFU we use.

Downtime is a tie — so what actually decides it?
With the HIFU-versus-laser choice, downtime is the headline difference. Here it isn't: neither RF nor HIFU breaks the vaginal surface, so both are essentially no-downtime, with the most common after-effects being mild transient warmth, brief tenderness, and occasional light spotting in the first day. That means the decision between these two rests on three other things — how precisely the energy is placed (HIFU targets set depths; RF heats more broadly), how many sessions you'll realistically need (HIFU is often 1–2; premium RF is frequently a series), and what it costs to reach your goal (covered below).
Results and the honest evidence picture
Both approaches work through the same final pathway — your body's collagen and elastin response — and both produce moderate, gradual firming rather than dramatic, surgical-level change. Improvement develops over roughly 6–12 weeks and continues for a few months, then remodels over time, which is why neither is permanent and why maintenance is part of the plan.
It's worth being straight about the evidence. Energy-based vaginal procedures are an area of active study, and the published literature is still maturing for both RF and HIFU. The U.S. FDA has cautioned that energy-based devices marketed for vaginal "rejuvenation" are not cleared specifically for these cosmetic indications. That doesn't mean the treatments don't help — many patients report real satisfaction — but it does mean any clinic promising guaranteed or dramatic results is overselling. A trustworthy consultation sets realistic expectations and screens you properly first.
Who each one tends to suit
HIFU is often chosen when the goal is targeted firming at depth with the fewest sessions, and when you'd prefer precise energy placement. RF is sometimes chosen where a gentler, broad warming is preferred, or where treating the external labial area in the same visit matters, since some RF platforms do both. In practice the decision is individual — it depends on your history, your examination, and what you're actually trying to change. When neither is the right answer
Both HIFU and RF are for mild-to-moderate concerns. Severe laxity from significant tissue loss, severe stress incontinence requiring surgical correction, and pelvic organ prolapse are not treated by energy-based devices — they need a surgical or specialist referral, and we'll tell you that directly rather than sell a session that won't deliver. For post-childbirth mild bladder leaks, pelvic-floor training comes first and energy treatment is at most an adjunct. Active infection, pregnancy, and untreated dysplasia are reasons to defer or screen further first.
Cost, and how to compare fairly
This is where the two diverge most.
