PEMF vs TENS — what's the difference?

They both promise pain relief from a small device, so they get confused constantly. But PEMF and TENS work in completely different ways and do different jobs. Here's the plain-language difference — and how to think about which, if either, fits your problem.

Short answer: TENS sends a mild electrical current through skin pads to mask or block pain signals — fast, felt-as-a-tingle, but temporary symptomatic relief. PEMF uses magnetic-field pulses to support the tissue and reduce inflammation over a course — slower and more of a recovery adjunct. Think of TENS as a pain "volume knob" and PEMF as background support. Neither fixes the underlying cause, and for most problems, movement and strengthening matter more than either device.

How TENS works

TENS stands for transcutaneous electrical nerve stimulation. Sticky electrodes on the skin deliver a mild electrical current you feel as a buzzing or tingling. The idea is straightforward: that stimulation interferes with pain signals travelling to the brain (and may prompt some release of the body's own pain-dampening chemicals), so the pain feels quieter while the unit is on. It's genuinely useful for symptomatic relief — a way to turn the volume down during a flare — but the effect is temporary and it isn't changing the tissue or the cause.

How PEMF works

PEMF — pulsed electromagnetic field therapy — is a different mechanism entirely. Instead of electrical current through the skin, it delivers pulses of a low-frequency magnetic field that are thought to influence cell activity and local microcirculation, supporting the tissue's own anti-inflammatory and repair processes. The aim isn't to mask pain in the moment but to support recovery over a course, which is why it's used as an adjunct across several weeks rather than as instant relief. Which is right for what

Because they do different things, "which is better" is the wrong question. TENS shines as a home tool for turning down the volume of nerve-type or muscular pain when you need to get through the day. PEMF is used as part of a recovery plan — most often in the clinic alongside radial shockwave and, above all, a movement and strengthening program. In practice, they're complementary rather than competing, used at different moments. And the honest headline applies to both: neither is a cure, and for most persistent pain the biggest gains come from diagnosing the cause and rebuilding load tolerance.

Home devices, and a word of caution

TENS units are inexpensive and, once you know your diagnosis, can be a reasonable home aid. Home PEMF devices are a different market — power and quality vary enormously, and cheap consumer mats deliver far weaker fields than clinical systems while carrying big wellness claims.
